There are two registered nurses available that perform care management for patients. Care management is a service for patients to help them navigate the healthcare system. It can be calling you after you have been in the hospital to see how you are doing, making sure you can afford your medications and getting you help if you can’t, helping you manage chronic illnesses like diabetes and hypertension and helping you get services in your home if and when you need them. This is a small sample of what care managers do. The best thing about care management is that it is always free.
